Jada Brooke Van Hook, 20, of Sidney, passed away on June 15, 2025, in Sidney, Ohio. She was born on September 10, 2004, to Darryl Van Hook and Jacqui Tobin.
She is also survived by her daughter - Everly Moton, her sisters - Jaylee McDaniel and Chloe Riegel, as well as her grandparents, aunts, uncles, and several cousins.
A viewing will be held at Salm - McGill & Tangeman Funeral Home on Monday - June 23, 2025, from 5 PM - 7 PM. Services are in the care of Salm - McGill & Tangeman Funeral Home - Sidney.
